Metal: Hellsinger to połączenie pierwszoosobowej strzelanki z grą rytmiczną, w której przemierzając rejony Piekła, zabijamy hordy potworów w rytm metalowej ścieżki dźwiękowej.

W grze wcielamy się w Niewiadomą, pół demonicę, pół człowieka, która napędzana żądzą zemsty, przemierza coraz niższe rejony Królestwa Koszmarów (potocznie zwanego Piekłem), by na samym ich końcu spotkać się twarzą w twarz z Czerwoną Sędziną. Zanim to jednak nastąpi przed nią mnóstwo potyczek z hordami demonów i ich plugawych dowódców zamieszkujących piekielne czeluści, na które, wbrew potocznym wyobrażeniom, składają się nie tylko ogniste pustkowia, ale też mroczne lasy, lodowe krainy i inne pokręcone lokacje.
